The 5 Ways Hepatitis B Actually Spreads
1. Sexual contact. Unprotected vaginal, anal, or oral sex with an infected partner is one of the most common transmission routes in adults. The hepatitis B virus (HBV) is present in semen and vaginal fluids at concentrations high enough for transmission. According to the CDC, sexual transmission accounts for approximately two-thirds of acute hepatitis B cases in the United States.
2. Blood-to-blood contact. Sharing needles, syringes, or other injection equipment is a high-risk transmission route. This includes injection drug use, tattooing or piercing with non-sterile equipment, and accidental needle-stick injuries in healthcare settings. HBV can survive outside the body on surfaces for up to 7 days, remaining potentially infectious during this period.
3. Mother-to-child transmission (vertical/perinatal). Without intervention, mothers with hepatitis B have a 70-90% chance of transmitting the virus to their newborn during delivery. What to Do After a Potential Exposure
If you have been exposed to hepatitis B — through a needle-stick injury, unprotected sex with a known HBV-positive partner, or other high-risk exposure — time-sensitive post-exposure prophylaxis (PEP) is available:
Hepatitis B immunoglobulin (HBIG) — provides immediate passive immunity and should be administered within 24 hours of exposure (ideally within 12 hours). Effectiveness decreases significantly after 7 days.
Hepatitis B vaccine series — should be initiated simultaneously with HBIG if the exposed person is unvaccinated. The vaccine triggers active immunity that develops over weeks.
